Appointments
We make every effort to collect as much information as possible when booking your appointment in order to ensure a smooth check-in process. You will not need to arrive early for your appointment.
What to bring to every appointment
- Insurance card(s)
- Prescription insurance card (if different from above)
- Co-pay
- Translator, if you do not speak English
What to bring to your first appointment
- Patient registration form
- Medical history
- Permission to treat minors, if applicable
- Paperless-billing credit card authorization form, if participating
We respect that your time is valuable, and we make a sincere effort to adhere to the office schedule. Nevertheless, delays are sometimes unavoidable. When this happens, your patience is appreciated.
Out of courtesy, if you cannot keep an appointment, please give our office 24 hours notice.
In case an urgent appointment is needed, please explain the circumstances to the receptionist, and we will make every effort to see you as soon as possible.
